Job summary

Health Connect America in the United States seeks a Family Development Specialist to support clients and families by developing and implementing treatment plans, providing therapeutic interventions, and linking them to needed services.

The role emphasizes collaboration with the treatment team, coordination of care, timely documentation, and outreach to recruit referrals while ensuring compliance with COA standards.

Qualifications

• Bachelor’s degree in a human services discipline such as Social Work, Psychology, Counseling, Sociology, Criminal Justice, or a related field.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapy interventions and skill-building as outlined in the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with services and resources identified in the treatment plan.
• Serve as a liaison with professionals, agencies, and community resources to meet client needs.
• Maintain accurate documentation of client interactions, treatment plans, and progress notes in line with policy and standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training and professional development to stay current with best practices.

Education

Bachelor's degree in a human services discipline such as Social Work, Psychology, Counseling, Sociology, Criminal Justice, or a related field

• Bachelor’s degree in a human services discipline such as Social Work, Psychology, Counseling, Sociology, Criminal Justice, or a related field, is required.
• Minimum of two years experience working with children and/or families. This experience may include internships and volunteer work.
