Job summary

Alignment Health is seeking a compassionate, organized Bilingual Spanish Health Coach & Educator to support seniors across Maricopa County, AZ. This field-based role conducts in-home visits, builds trusted relationships with members and families, and provides wellness coaching to improve health outcomes.

You will travel locally, manage a schedule of member visits, and receive mileage reimbursement. Must be bilingual Spanish and proficient in healthcare privacy guidelines.

Qualifications

• Strong communication with patients, caregivers, and clinicians.
• Ability to work independently in the field and manage a daily schedule.

Responsibilities

• Contacts patients eligible for Alignment’s Care at Home program and provides wellness coaching.
• Collaborates with clinicians/social workers to develop action plans.
• Encourages healthy habits to improve quality of life.
• Delivers high level of patient service to increase retention and satisfaction.
• Assists patients in accessing community resources and social services.
• Develops and distributes health education materials to patients, caregivers, and families.
• Coordinates care with Alignment team members and external vendors.
