Job summary

Alignment Health is seeking a bilingual Spanish Health Coach and Educator to conduct home visits in Los Angeles/Orange County, CA, with mileage reimbursement. The role blends in-person and telehealth work within the Care Anywhere team to support seniors and high-risk patients.

Qualifications

• Required: 1+ year experience as wellness/health coach or educator
• Proven ability to educate patients and families in plain language
• Able to conduct home/telehealth visits with sensitivity and confidentiality

Responsibilities

• Conduct 6–8 home visits per day with mileage reimbursement
• Develop wellness coaching plans in collaboration with the care team
• Provide health education materials to patients, caregivers, and families
• Coordinate care with clinicians and social workers to support patient health
• Assist patients in accessing community resources and social services
• Engage in remote work via telehealth and phone to support ongoing care
