Job summary

Hamilton County Health & Social Services Division seeks a professional to provide social counseling, care coordination, support, and case management across programs addressing prevention, education, rehabilitation, and self-sufficiency.

The position requires bilingual proficiency in English and Spanish, a bachelor's degree in related field, and at least one year of direct social services experience. Attendance is in-person with occasional on-call duties and potential irregular hours.

Qualifications

• Bachelor’s Degree in Social Services, Nursing, Human Services, Early Childhood Education, Counseling and/or Behavioral Sciences or related field and one year of direct work experience in Social Services.
• A valid driver’s license is required.

Responsibilities

• Provide professional social counseling, care coordination, support, linkage to care, and/or clinical case management.
• Assess individuals or families, develop care plans, and refer to community resources.
• Maintain accurate documentation and liaise with community partners.
• Conduct community outreach and home visits as needed.
