Salary: Bachelors degree: Starting Salary: $26.75-$29.48 hourly
Full Salary Range: $26.75-$32.55

Master’s degree: Starting Salary: $30.98-$34.14
Full Salary Range: $30.98-$38.73

*Pay rate is based on level of experience equivalent to TCRC

+$110 biweekly bilingual stipend if applicable

Job Title: Service Coordinator II

Job Summary: Within a team environment, provides person-centered consultation, advocacy, resource information and service coordination for people with developmental disabilities who require more involved and specialized assistance. The position will collaborate with the Person-Centered Practices Specialist to ensure knowledgeable and use of person-centered planning, and how it applies to supporting this specific group of people and families. This position will collaborate with the Benefits Specialist and the Multi-Cultural Specialist to ensure they are using innovative approaches to service coordination and accessing generic resources.

Specific Job Duties:
  • Manage a 1:40 caseload ratio currently served by TCRC with the low POS and/or no POS
  • Utilizing a Person-Centered approach to the Individual Program Plan Process, will work to identify barriers is accessing services and supports; to enhance relationship between individual/family and TCRC; to empower individuals and families to partner with TCRC and other community agencies and to assist in identification of new resources that are culturally relevant to their community.
  • Works in collaboration with the PCP Specialist, Cultural Specialist, Self-Directed Services team and other TCRC Departments to identify innovative approaches to service coordination.
  • Using PCP strategies will be responsible for the development and implementation of the individual’s IPP, progress reports, annual reviews, and quarterly in person reviews.
  • Provides follow-up, reassessments, and periodic review of individual’s progress and program effectiveness
  • Adheres to the agency mission, philosophy and core values
  • Assists in locating and developing natural supports.
  • Initiates the purchase of vendored and generic services and closely monitors service delivery. As well as assists in navigating state-wide systems such as Departments of Managed Health Care, Social Services and Rehabilitation.
  • Keeps abreast of state and federal regulations, agency policy and procedure and guidelines related to services for individuals. Acts as an advocate on social, legal, education, mental health and/or forensics issues related to the individual.
  • Participates in outreach and represents the agency in community meetings and events
  • May conduct public presentations regarding regional center services and activities
  • May serve on intra-agency and/or inter-agency
  • May participate in data gathering and analysis activities
  • Additional duties may be assigned as deemed appropriate(i.e. community presentations, attending local resource fairs, etc.)
Primary Job Functions:
  • Conduct strength-based assessments that communicate an understanding of the unique issues facing family members.
  • Involve individuals we serve and family in all aspects of person-centered planning and support activities
  • Design, document, implement and monitor services and supports in a timely manner
  • Routinely contact individuals and families to answer emerging questions, quarterly in-person review meetings and annual IPP or IFSP meetings.
  • Facilitate planning team progress, identifying outcomes, needed services and Ongoing data collection.
  • Develop and maintain a network of vendors and community agencies
  • Provide relevant information and training about developmental disabilities, services and resources
  • Use effective communication and interpersonal skills that consistently respect children and adults with developmental disabilities and their families of varying cultural backgrounds.
  • Demonstrate an operative knowledge of family/natural support and private/public resources and integrate these into service planning resulting in community-based outcomes.
  • Demonstrate an operative knowledge about developmental disabilities and how families are impacted
  • Demonstrate an operative knowledge of a variety of approaches to intervention and support
  • Demonstrate an operative knowledge of entitlement and benefit programs.
  • Demonstrate an operative knowledge of the Tri-Counties Regional Center system, the legal system, individual civil rights and overall advocacy.
  • Work in a cooperative and collaborative manner as a team member
  • Develop and maintain good relationships with community representatives
  • Participate in professional growth and development through attendance at in-service and other training activities, conferences and other job-related agency approved events.

Knowledge, Skills, and Abilities Required:

  • Knowledge of developmental disabilities
  • Demonstrate cultural awareness and sensitivity and an ability to work effectively with culturally diverse populations in a variety of situations including excellent group facilitation skills
  • Awareness of entitlement and benefit programs and of public benefit programs

QIDP required

Valid California driver’s license and reliable, insured vehicle or equivalent means of transportation. This position will require regular travel outside of their home office’s catchment area, and can include occasional travel to state or national meetings.

Minimum Qualifications:
  • Master’s or Bachelor’s degree in human services field or a related area, with 2 years of applicable experience in mental health or social work.
  • 2 years experience case management within a related field
  • 1 year experience with individuals with developmental disabilities
  • Experience in using Person Centered Thinking (PCT) and Person-Centered Planning (PCP)
  • Bilingual and bicultural English/Spanish – Required
  • Professional and lived experience in working with families from diverse cultural, linguistic, and socio-economic backgrounds.
Physical Requirements:

Position requires the ability to work independently. Must possess mobility to work in a standard office setting and use standard office equipment, including a computer. This is primarily a sedentary office classification. The position also requires grasping, repetitive hand movement, finger dexterity and fine coordination in preparing statistical reports and data using a computer keyboard and calculator. Additionally, the position requires near, far, and color vision in reading correspondence, statistical data, and using the computer. Hearing and speech are required to communicate in person, before groups and over the telephone.

Employees must possess the ability to lift, carry, push and pull materials and objects necessary to perform job functions. The employee must frequently lift and/or move up to 10 lbs. and infrequently up to 20 lbs. The position requires standing, walking, reaching, twisting, turning, kneeling, bending, squatting, and stooping in the performance of daily office activities. Work is performed in an office setting with moderate noise levels, controlled temperature conditions and no direct exposure to hazardous physical substances. Employees may encounter upset individuals over the phone and/or in person. Each of these physical tasks is an essential function of the position.
