Job summary

The Center for Human Development (CHD) in Springfield, MA seeks a Registered Nurse for the Adult Community Clinical Services (ACCS). The RN will coordinate health services, perform assessments, provide counseling, and liaise with medical and community providers to promote wellness for Persons served.

Qualifications

• MA RN license required
• Bilingual English/Spanish candidates required
• Two years of experience in mental health and/or substance use field preferred
• MAP experience preferred
• Residential experience preferred
• Acceptable driving record

Responsibilities

• The RN coordinates those functions and activities that provide for, maintain and promote the health of Persons served.
• The RN provides direct support to Persons to assess needs and participates in medical reviews.
• The RN provides counseling in areas of health and psycho-physiological well-being and liaises with other medical and community-based providers.
• This is an administrate RN role.
