Job summary

Pair Team is seeking a highly motivated full-time Lead Care Manager to advance our whole-person, interdisciplinary care model for individuals with serious mental illness, SUD, homelessness, or high medical needs.

The role is primarily remote, with 2–3 on-site community visits per month near your city, while most duties are performed from home. You will engage with the team to promote timely, personalized care and trust-based relationships.

Qualifications

• 1+ years of general work experience (Case Management preferred).
• You are physically located in California.
• Bilingual English/Spanish required.
• Strong cultural fluency and empathy.
• Ability to collaborate in a multidisciplinary team.
• Strong organizational skills and ability to stay patient under adversity.
• Comfort with technology, CRM databases, and basic MS Office tools.
• Valid driver’s license and auto liability insurance.

Responsibilities

• Maintain ongoing caseload of individuals using evidence-based approaches to promote engagement and health goals.
• Use relationship-based strategies to support members with social navigation and trust-building.
• Conduct periodic telephonic and SMS outreach to ensure timely follow-ups.
• Work with members to identify health and wellness goals and incorporate them into action plans.
• Support other care team members with delegated tasks.
• Collaborate on care issues with the Enhanced Care Management team and clinicians before clinical actions.
• Meet monthly encounter metrics to ensure compliance with health plan regulations.
• Identify and remove barriers to continued program participation.
• Assist individuals in securing connections to community supports and referrals.
• Coordinate physical care management appointments with providers.
• Utilize online platforms to collaborate with team members and complete daily tasks.
