Qualifications

• Bachelor’s degree in nutrition or related field.
• ACEND-accredited supervised practice program.
• Passed the CDR national examination.
• Bilingual English/Spanish preferred.
• Six months practical experience in the field or waiver option.

Responsibilities

• Make appointments for patient follow-up visits and enter progress notes in EHR (POMR format).
• Refer patients to appropriate providers when needed.
• Coordinate biweekly Food Share mobile food site visits.
• Collect food from community resources for demonstrations and food pantries.
• Plan and coordinate weekly nutrition education classes on health topics.
• Provide individual counseling sessions on dietary issues as referred by Provider.
• Develop and coordinate nutrition education materials and demonstrations.
• Prepare reports for Chief Medical Officer as requested.
