Job summary

Astrana Health is seeking a skilled Primary Care Physician to join our outpatient clinics in Alhambra and Whittier, CA. This role emphasizes preventive medicine, chronic disease management, and value-based care.

Ideal candidates are bilingual in Spanish or Chinese and have strong patient-centered care skills. This is a full-time, on-site position with a competitive compensation range and sign-on/retention bonuses depending on location and experience.

Qualifications

• MD or DO from an accredited medical school.
• Completion of an accredited Family Medicine or Internal Medicine residency program.
• Active, unrestricted California Medical License.
• Active DEA registration.
• Current BLS and ACLS certifications.
• New graduates will be considered.

Responsibilities

• Provide and manage comprehensive primary care services, including physical examinations, evaluations, diagnoses, counseling and treatment.
• Preventive care visits, wellness exams, and age-appropriate screenings.
• Manage chronic conditions such as diabetes, hypertension, COPD, heart disease, and other common adult medical conditions.
• Prescribe medications and treatment regimens per clinical guidelines.
• Develop evidence-based care plans with patients and families.
• Maintain continuity of patient panels with follow-up and long-term care planning.
• Record and maintain accurate EHR documentation.
• Order, interpret, and follow up on diagnostic tests, labs, and imaging.
• Collaborate with specialists and behavioral health providers to coordinate care.
• Direct and collaborate with nursing staff for efficient patient flow.
• Participate in quality improvement initiatives including HCC, 5-Star, and incentive programs.
• Advocate for cost-effective, coordinated care.
• Suggest workflow improvements to enhance efficiency and patient experience.
• Uphold medical ethics and confidentiality.
• Other duties as assigned.
