Responsibilities

• Provide parent education to assist families with in-home structure.
• Contact service providers to gather ongoing information on clients' needs.
• Assist clients in obtaining grants and benefits.
• Assist with case management and administrative tasks using electronic health record.
• Participate in case reviews and agency-wide training.
