Zufall Health in New Jersey is seeking a Prenatal Case Manager to support prenatal patients and families, coordinating access to medical and social services. The role collaborates with the SVP of Nursing and Clinical Operations and the Prenatal Care team to address barriers such as insurance, language, and transportation.

Travel within local area is required, and the position emphasizes compassionate, trauma-informed care, documentation in eCW, and referral to charity care and community programs

Formación

• Bachelor's degree in health or social services related field.
• Minimum two years of clinical experience.
• Experience with Medicaid/insurance processes.
• Proficiency in medical terminology.
• Bilingual English/Spanish required.

Responsabilidades

• Identify prenatal patients needing services and coordinate care with the team.
• Meet with patients and conduct follow-up visits as needed.
• Document services and update plans of care in the EMR.
• Arrange transportation, translation, and other barriers to attendance.
• Assist with charity care and benefits applications when appropriate.
• Maintain HIPAA-compliant records in eCW and care notes.
• Support referrals for newborns and community resources.
