Job summary

The role emphasizes addressing psychosocial needs, discharge planning, and community linkages to support pediatric patients and their families. Requirements include a Master’s degree in Social Work and IL licensure (LSW), with LCSW preferred, and Spanish language abilities are strongly preferred.

Qualifications

• Master's degree in social work is required.
• Current Illinois State Social Work licensure (LSW) required; LCSW preferred.
• Experience in health care, child welfare or mental health settings preferred.
• Spanish language abilities strongly preferred.

Responsibilities

• Coordinate service delivery with families and care teams across the continuum.
• Meet daily with inpatient case management to review census and discharge barriers.
• Assess and respond to child/family needs including developmental and cultural aspects.
• Develop relationships with families to support patient well‑being and treatment.
• Provide timely intervention for emergencies and crisis situations.
