Qualifications

• High school diploma or equivalent.
• 1+ years of customer service experience.
• 1+ years of home healthcare experience preferred.
• Bilingual English/Spanish required.

Responsibilities

• Receive and process referrals from hospitals, physicians, and other healthcare providers.
• Collaborate with internal teams to determine patient eligibility and initiate intake.
• Serve as the primary point of contact for patients, families, and healthcare professionals.
• Address inquiries and provide guidance on accessing services and resources.
• Schedule patient appointments, coordinating with healthcare professionals and patient preferences.
• Monitor and update schedules to accommodate changes, cancellations, and rescheduling.
• Communicate pertinent patient information to appropriate individuals.
• Maintain accurate records of patient information and communications.
• Provide compassionate and empathetic support throughout care coordination.
