Job summary

Responsibilities

• Conduct ongoing community outreach to recruit participants into the program.
• Complete intake, program screeners, and/or assessments to determine other recommendations or support services for the client.
• Utilize client and family input to provide ongoing case management for fathers enrolled in the program.
• Develop a plan of service outlining services and interventions from DePelchin and the community.
• Assist clients with referrals and advocacy for entitlements or needed services.
• Maintain current records and meet agency recording requirements.
• Maintain monthly statistics and data for accountability and billing.
• Provide curriculum-based groups to families in the community.
