Qualifications

• CPR/BLS certification required.
• Spanish bilingual with oral/written assessments.
• High energy with dynamic presentation skills.

• Understand clinical targets and outcome measurements; develop action plans.

Responsibilities

• Provide onsite face-to-face health coaching and support.
• Identify health education needs via targeted assessments.
• Collaborate to establish health improvement plans and goals.
• Empower clients to engage in their health outcomes.
• Lead wellness activities, group coaching, and seminars.
• May perform biometric screenings (finger sticks, BP, etc.).
• Use biometric data and motivational interviewing to drive outcomes.
• Support site events like open enrollment and health fairs.
