Job summary

Clover Health is seeking a compassionate Nurse Practitioner for the New Jersey home-care team. The hybrid role involves in-home visits and telehealth care for Medicare Advantage members in Bergen County, with Korean and English language fluency essential for effective communication with members and caregivers.

You will conduct comprehensive assessments, manage chronic conditions, and develop personalized care plans that address clinical and social determinants of health.

Qualifications

• Active NP license in New Jersey or ability to obtain.
• National board certification as Nurse Practitioner.
• Active DEA and New Jersey CDS registration, or ability to obtain.
• Fluency in Korean and English for clinical assessments and education.
• Experience caring for adult or geriatric populations in primary/complex care or home-based settings.

Responsibilities

• Conduct in-home and telehealth visits for Medicare Advantage members.
• Provide care and education in Korean and English to members and caregivers.
• Perform comprehensive assessments, medication reviews, and chronic disease management.
• Develop individualized care plans addressing clinical and social determinants of health.
• Identify barriers to care including fall risks, food insecurity, caregiver challenges, and gaps in treatment adherence.
• Collaborate with physicians, care coordinators, social workers and other team members.
• Leverage Clover Assistant and EMR to support proactive care and close gaps in care.
• Monitor health status and intervene early to prevent avoidable ED visits and hospitalizations.
• Educate members and caregivers on disease management and preventive care.
