Job summary

Clover Health is seeking a compassionate Nurse Practitioner fluent in Korean and English to join our New Jersey Home Care team. The role blends in-home visits with telehealth, delivering patient-centered care to Medicare Advantage members in Bergen County and surrounding areas.

You will perform comprehensive assessments, manage chronic conditions, and educate patients and caregivers in both languages, while coordinating with a multidisciplinary team to close care gaps.

Qualifications

• Active NP license in New Jersey or ability to obtain.
• National board certification as a Nurse Practitioner.
• Active DEA and NJ CDS registration, or ability to obtain.

Responsibilities

• Conduct in-home and telehealth visits for Medicare Advantage members.
• Provide care and education in Korean and English.
• Perform comprehensive assessments and chronic disease management.
• Develop and implement individualized care plans addressing clinical and social determinants.
• Identify barriers to care such as fall risks, food insecurity, and adherence gaps.
• Collaborate with physicians, care coordinators, social workers, and other team members.
• Utilize Clover Assistant and EMR to support proactive care and close gaps.
