Job summary

Greater Lawrence Family Health Center, Inc. seeks a bi-lingual (Spanish/English) Nurse Practitioner to provide high-quality care to our diverse patient population. Reporting to the Medical Director, the NP will assess patients and help manage chronic illness, acute episodes and preventive care.

The role requires at least 2 years of NP experience, MA licensure, teamwork, and a willingness to teach others. Travel between multiple GLFHC sites may be required. Bilingual Spanish is required.

Qualifications

• At least 2 years of recent Nurse Practitioner experience.
• Licensed as a Nurse Practitioner in Massachusetts.
• Works well within a multi-disciplinary team.
• Bi-lingual in Spanish and English.

Responsibilities

• Makes assessments of patients including history, meds, labs, psychosocial issues and living arrangements.
• Reviews abnormal labs and plans disposition/treatment.
• Identifies education needs and develops teaching plans with patients and families.
• Collaborates with case managers and other providers to coordinate care.
• Provides care following approved protocols and documents patient education.
• Travel between sites may be required.
