Job summary

Suvida Healthcare is seeking a Clinical Informatics Specialist to ensure accurate and efficient medical documentation during patient encounters. You will work with primary care providers to scribes, record histories, exams, tests, and plans into the EHR, enabling clinicians to focus more on patient care.

You will document, review charts, and coordinate with the care team, while maintaining HIPAA compliance and accuracy in coding and data entry across multiple Neighborhood Centers in Houston, TX.

Qualifications

• Bachelor's degree in healthcare or related field.
• Minimum of 2 years of clinical experience.
• Familiarity with medical terminology, anatomy, and physiology is a plus.
• Prior experience as a Medical Scribe is preferred, though not obligatory.
• Ability to efficiently use electronic health record (EHR) systems.
• Strong typing skills and advanced computer proficiency.
• Effective written and verbal communication skills.
• Ability to thrive in a fast-paced clinical environment with a commitment to accuracy.
• Strong organizational skills, effective time management, and problem-solving capabilities.
• Willingness to move between Neighborhood Centers as needed.
• Proficiency in both English and Spanish is required.

Responsibilities

• Document patient encounters accurately during visits and record all relevant information in real time.
• Enter diagnoses, procedures, and notes into the EHR ensuring accuracy and privacy compliance.
• Review and organize patient charts to ensure completeness and correctness.
• Act as liaison between care team members to facilitate information exchange.
• Maintain accurate ICD-10 and CPT coding for encounters and services.
• Support documentation queries between coding team and providers.
• Prepare data for morning huddle by researching historical charts and external data.
• Oversee quality metrics—monitor, analyze, and report to close care gaps.
• Support population health initiatives including patient stratification and chronic care management.
• Stay updated on evidence-based protocols and provide updated data to the care team.
