Job summary

Asian Health Services seeks a Health Coach II (Medical Assistant) to support the chronic care management team. Under supervision from the Clinic Site RN Manager, you will assist providers, coordinate patient care, and perform clinical tasks with an emphasis on culturally competent, patient-centered care.

You will take vitals, document medical history, enter data into the EHR, perform basic in-clinic tests, and help manage patient flow while maintaining privacy and safety.

Qualifications

• Bilingual in Cantonese/Vietnamese required
• Strong communication skills with patients and team required

Responsibilities

• Takes and records vital signs, recognizing abnormalities and reporting to nurses and providers.
• Interviews patient to obtain medical history using SOAP format.
• Enters health information into EHR and retrieves reports as needed.
• Performs basic in-clinic lab tests and specimens processing.
• Assists in clinical patient care and coordination of referrals as directed.
