The Care Coordinator will contribute to the care and well-being of SCAN members, empowering them to age in place by collaboratively assessing, planning, coordinating, and monitoring their holistic needs. Focus on addressing members' care coordination needs, processing referrals and services, and supporting navigation of the healthcare system.
