Job summary

La Clinica de Familia (LCDF) in Southern New Mexico is seeking a co-located, collaborative integrated care behavioral health provider to join the PCMH care team. The BHP will deliver brief consultation-based services within the primary care clinic, collaborating with PCPs and multidisciplinary staff to improve recognition, treatment, and management of psychosocial issues.

Must be bilingual in English/Spanish; requires Master's in Social Work or related field and NM licensure

Qualifications

• Master's Degree in Social Work or related field.
• Must hold NM LMSW/LMHC/LCSW/LPCC/LMFT license.
• Bilingual English/Spanish required.
• Experience with brief clinical interventions in primary care setting.

Responsibilities

• Provide consultation to patients referred by PCP.
• Deliver brief, integrated care services in primary care clinic.
• Collaborate with MH team to improve psychosocial care in PCP setting.
