Job summary

Fair Haven Community Health Care seeks an Intake Therapist to provide crisis emergency care and comprehensive psychosocial assessments within a community behavioral health setting. The clinician will triage emergencies, perform risk and diagnostic assessments, and develop crisis treatment plans while coordinating with BH navigators and local services.

Evening and Saturday hours are required. Candidates must hold a Master’s in clinical behavioral health and be license-eligible in Connecticut;

Qualifications

• Be bilingual in English and Spanish.
• Have clinical skills in assessment and diagnoses across a broad range of presentations and ages.
• Demonstrate knowledge of psychopathology, crisis theory and behavioral health interventions.
• Able to function independently and seek guidance when needed.
• Experience with culturally diverse urban populations and community resources.
• Knowledge of DSM-5/DSM-5-TR and related diagnostic practices.
• Licensed or license-eligible in Connecticut (LMSW, LCSW, LMFT, LPC, PsyD, or PhD).

Responsibilities

• Triages clinical emergencies and applies safety protocols.
• Conducts telephone triage, risk assessments, de-escalation, and safety planning.
• Facilitates referrals to emergency departments and community services.
• Completes crisis and diagnostic assessments for adults and children.
• Develops crisis treatment plans integrating community health and social services.
• Completes documentation and enrollment forms in a timely manner.
• Collaborates with BH navigators, directors, and clinicians for smooth transitions.
• Uses evidence-based screening tools (e.g., GAD, PHQ-9, CSSR, CAGE, SDOH).
• Coordinates with Billing on insurance issues and payment options.
