Job summary

CN Guidance & Counseling Services, Inc. seeks a dedicated coordinator to provide individualized coaching, support, and coordination for Project FORWARD participants.

You will conduct initial and ongoing assessments to identify needs, goals, and resources, and develop each participant's Individual Development Plan in collaboration with facilitators and other service providers. The role requires effective outreach, engagement with community organizations, and documentation for local and federal

Qualifications

• Bachelor's degree in human services, social work, mental health counseling, or related field; Master's Degree preferred.
• Experience in a mental health related field, coordination of services, employment/education services, completing collateral contacts.
• Bilingual in English and Spanish preferred.

Responsibilities

• Conduct outreach to recruit/engage potential participants and community organizations.
• Conduct intakes for potential participants with Facilitator.
• Perform initial and ongoing assessments to determine strengths, needs, goals, and resources.
• Maintain relationships with participants and serve as a consistent point of contact.
• Create and monitor each participant's Individual Development Plan and document progress.
