Qualifikationen

• Three years of full-time or equivalent experience in human services or social work.
• Bachelor's degree may substitute for two years of required experience.

Aufgaben

• Monitor safety and well-being of assigned individuals.
• Provide advocacy in human, civil, and legal rights; facilitate team meetings and maintain records.
• Maintain regular contact with individuals, service providers and families.
• Organize crisis intervention and follow-up planning; inform supervisor and Area Office team.
• Assist with Participant Directed Services as needed.
