Job summary

Brockton Home Health Care Agency LLC is seeking a licensed RN to lead patient care in home health settings, using Oasis to admit, transfer, re-certify, and discharge patients per physician orders. You will coordinate and supervise LPNs and HHAs, while maintaining compliance with agency policies and Medicare/Medicaid regulations.

Ideal candidates will have at least one year of RN experience and bilingual skills (Portuguese, Cape Verdean Creole, and Spanish) to support diverse patient populations

Qualifications

• Be a registered nurse with a current license.
• Minimum of one year of nursing experience as an RN.
• Bi-lingual preferred (Portuguese, Cape Verdean Creole and Spanish)

Responsibilities

• Perform initial home care patient visit and re-evaluate patient needs and progress regularly.
• Initiate the plan of care under doctor’s orders.
• Perform admission, transfer, re-certification, resumption of care, and discharge Oasis for home care patients.
• Observe, assess, and document symptoms.
• Monitor patient progress and reactions.
• Educate patients and caregivers on disease processes, medications, plan of care, and treatment plans.
• Coordinate patient services and supervise LPNs/HHA’s.
• Notify physician and clinical leadership of changes in patient condition.
• Ensure discharge criteria are met and update clinical records accordingly.
