Job summary

Qualifications

• Education: bachelor’s degree in public/community health education or related fields.
• Experience: two years as a community worker with outreach education in health.
• Bilingual English/Spanish proficiency preferred; ability to communicate across cultures.

Responsibilities

• Monitors and documents patient encounters; informs physicians of status.
• Provides individual/group health education and counseling as referred.
• Responds to inquiries about health education activities and services.
• Conducts patient assessments and surveys to identify health needs and program effectiveness.
• Develops relationships with community and government orgs to provide resources.
• Participates in developing and evaluating educational materials and sessions.
