Qualifications

• Minimum 1+ year of experience as a wellness health coach or educator.
• Current, valid driver's license and reliable personal transportation.
• Ability to travel throughout Maricopa County.
• Ability to maintain HIPAA compliance and confidentiality.
• Proficiency with Microsoft Office (Excel, Word, and PowerPoint).
• Associate degree or equivalent experience required.

Responsibilities

• Conduct approximately 6-8 in-home visits daily throughout your assigned service area.
• Provide wellness coaching and health education to support behavior change and healthier lifestyles.
• Partner with Advanced Practice Clinicians and Social Workers to develop member action plans.
• Connect members with community resources and social services.
• Support member engagement, retention, and satisfaction.
• Coordinate care with internal clinical teams and external partners.
• Deliver health education materials to members, caregivers, and families.

Alignment Health is seeking a compassionate, motivated Bilingual Health Coach & Educator to provide in-home support, wellness coaching, and health education to members throughout Maricopa County, Arizona. In this highly field-based role, you'll build trusted relationships with members, caregivers, and families while helping individuals adopt healthier habits, access community resources, and improve their overall well-being. What You'll Do

What to Expect

• Primarily field-based role with daily local travel.
• Monday-Friday, 8:00 AM-5:00 PM MST.
• Visits are scheduled geographically to maximize efficiency.
• Mileage reimbursement provided from door-to-door travel.
• Service areas include Scottsdale, Fountain Hills, North Phoenix, Paradise Valley, Cave Creek, Carefree, South Phoenix, Avondale, Tolleson, Laveen, Buckeye, Goodyear, and Central Phoenix.

Minimum Requirements

• Experience: Minimum 1+ year of experience as a wellness health coach or educator
• Current, valid, unrestricted driver's license.
• Reliable personal transportation and ability to travel throughout the assigned territory.
• Ability to maintain automobile insurance in accordance with company requirements.
• Ability to work independently and remotely.
• Strong communication, organizational, time management, and relationship-building skills.
• Proficiency with Microsoft Office (Excel, Word, and PowerPoint).
• Knowledge of health, wellness, nutrition, exercise, and behavior change principles.
• Ability to maintain HIPAA compliance and confidentiality.
• Education: Associate's degree or two years additional experience in lieu of education.

Pay Range

$44,987.00 - $67,480.00 Pay range may be based on a number of factors including market location, education, responsibilities, experience, etc.
