Alignment Health is seeking a compassionate bilingual Spanish Health Coach & Educator to support members through in-home, telehealth, and telephonic visits across Los Angeles and Orange County. You will coach on wellness, educate, and connect members with resources, while collaborating with clinicians and social workers to create personalized plans.

Responsibilities

• Conduct home and virtual visits with Alignment Health members.
• Provide wellness coaching to reduce high-risk behaviors and support healthy lifestyle changes.
• Collaborate with clinicians and social workers to create personalized action plans.
• Connect members with community resources and social services.
• Deliver health education and wellness materials to members, caregivers and families.
• Coordinate care with internal teams and community partners.
• Support member satisfaction, engagement and retention through exceptional service.
