Job summary

Alignment Health is seeking a compassionate bilingual Health Coach & Educator to provide in-home wellness coaching across Maricopa County, AZ. You will build trusted relationships with members, caregivers, and families, supporting healthier habits and connections to community services.

In this field-based role, you will conduct daily visits, collaborate with advanced practice clinicians and social workers, and travel throughout the assigned territory with mileage reimbursement provided.

Qualifications

• Minimum 1+ year of experience as a wellness health coach or educator.
• Current, valid driver's license and reliable transportation.
• Ability to travel throughout Maricopa County, AZ for in-home visits.
• Proficiency with Microsoft Office (Excel, Word, PowerPoint).
• Knowledge of health, wellness, nutrition, exercise and behavior change.

Responsibilities

• Conduct approximately 6-8 in-home visits daily within the service area.
• Provide wellness coaching and health education to support behavior change.
• Partner with clinicians and social workers to develop member action plans.
• Connect members with community resources and social services.
• Support member engagement, retention, and satisfaction; coordinate care with teams.
