Job summary

Commonpoint Queens is seeking a Community Health Outreach and Care Coordination professional to engage migrants and at-risk individuals, promoting participation in Connect2Care activities and coordinating referrals to health and social services.

You will support program administration, work closely with local organizations, and participate in health advocacy training to improve access to preventive care and wraparound supports.

Qualifications

• Bachelor's degree or equivalent in community health, case management, health coaching, or care coordination.
• Familiarity with the served community and strong system navigation skills.
• Bilingual in Spanish and English.
• Proficient with Google Workspace and new technology platforms.

Responsibilities

• Educate and build relationships with community members to promote participation in C2C activities.
• Screen individuals for health and social determinants of health (SDOH) needs and refer to appropriate services.
• Utilize the Connect UWNYC platform to track and manage referrals.
• Collaborate with healthcare stakeholders and local organizations to expand outreach.
• Support program administration and participate in training programs.
