Job summary

San Francisco AIDS Foundation is seeking a Health Access Treatment Connections Navigator to improve health outcomes for people who use drugs. This role will provide treatment connections, syringe access, overdose prevention education, and linkage to care. Collaboration with internal and external programs is essential.

The ideal candidate has at least 3 years of relevant experience and must be bilingual in Spanish and English. They will work in a supportive environment with comprehensive benefits that include 24 days of PTO in the first two years.

Qualifications

• Minimum 3 years experience working with people who use drugs, marginalized communities, or people experiencing homelessness.
• Knowledge of health, substance use treatment, and social services throughout San Francisco.
• Experience using motivational interviewing and care navigation required.

Responsibilities

• Connecting participants to internal and external substance use and medical services.
• Providing client-centered, relational proactive counseling.
• Documenting and entering all program data according to contract protocols.
