Qualifications

• Master's degree in Nursing or related field required.
• Board certification as Family Nurse Practitioner required.
• Bilingual in English and Polish required.
• 2–5 years of experience in Primary Care and Pediatrics preferred; new graduates welcome.

Responsibilities

• Provide comprehensive patient care: examine patients, obtain histories, perform exams, and develop diagnoses and treatment plans.
• Develop treatment plans and prescribe therapies within Illinois APRN scope.
• Collaborate with physicians and healthcare professionals to coordinate care.
• Order and interpret diagnostics, including labs, EKGs, and imaging as appropriate.
• Provide preventive and holistic care, counseling on health promotion and disease prevention.
• Perform clinical procedures such as injections, wound care, and immunizations.
• Coordinate patient care, follow up on referrals and results.
• Support the clinical team to ensure efficient patient care.
