Job summary

MedZed is seeking a Community Health Navigator II to provide field-based case management for higher-acuity Medicaid members in LA County. The role combines door-to-door outreach, assessments (PHQ-9 included), and care plan development with med plans and community resources.

This in-field position requires 4 days/week in the community, with hybrid/remote options. Requirements include a high school diploma and 2+ years in ECM or related.

Qualifications

• 2+ years in ECM, case management, or community health.
• Demonstrated excellence in documentation and compliance.
• Ability to manage complex psychosocial and medical needs.

Responsibilities

• Manage an ECM caseload up to 55+ members.
• Complete initial and follow-up ECM assessments including PHQ-9.
• Develop and update individualized Care Plans.
• Door knock and calls to enroll new members.
• Build and maintain a member panel through outreach.
• Coordinate referrals to health plans, providers, and community resources.
• Document timely, accurate case notes in Salesforce Health Cloud.
• Participate in interdisciplinary case conferences and team huddles.
• Escalate complex or high-risk member needs appropriately.
• Maintain compliance with CalAIM, DHCS, and health plan requirements.
