Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or related field.
• Master's degree in Social Work preferred.
• Minimum 2+ years of post-graduate experience in foster care, child welfare, family services, or case management.
• Knowledge of foster care services, family engagement, kinship support, trauma-informed practices, and community resources preferred.
• Proficiency with Microsoft Office and electronic case management or documentation systems required.

Responsabilités

• Family Engagement: Identify and engage relatives and other supportive individuals for placement resources.
• Needs Assessment: Assess caregivers and family circumstances for kinship placement opportunities.
• Placement Coordination: Support referrals, family meetings, pre-placement visits and related activities.
• Resource Navigation: Connect caregivers with community programs and services based on needs.
• Ongoing Support: Maintain communication with youth, caregivers and case teams; provide home-based support.
• Case Documentation: Maintain accurate case records and reports using case management systems.
