TAG MedStaffing is seeking a Youth Healthcare Support Specialist (Bilingual Spanish) in Bronx, NY to support adolescents and families in a community-based behavioral health setting. You will conduct home visits, coordinate care, and apply trauma-informed practices to ensure safe, positive outcomes.

Requirements include a Bachelor’s in a related field, 1+ year experience with youth or family services, and fluent English/Spanish communication.

Formación

• Bachelor’s degree in Social Work, Psychology, Human Services, or related field required.
• Bilingual English and Spanish required.
• 1+ years of experience with adolescents, youth services, foster care, behavioral health, or family support programs.
• Knowledge of trauma-informed care and youth development practices.
• Experience with case management documentation and reporting.
• Strong communication, relationship-building, organizational, and crisis management skills.

Responsabilidades

• Youth & Family Support: Conduct weekly in-person home visits with youth and foster families to support behavioral goals, life skills development, and daily care needs.
• Case Management: Maintain accurate case records, progress notes, assessments, behavior plans, and documentation related to client services and case activity.
• Crisis Intervention: Provide direct crisis support and collaborate with internal teams, foster parents, and external stakeholders to address urgent client needs.
• Behavioral Planning: Partner with youth, foster parents, and case workers to develop, implement, and monitor individualized behavioral goals and treatment plans.
• Community Coordination: Connect youth and families with educational, recreational, employment, and community-based resources to support long‑term development and stability.
• Trauma-Informed Care: Apply strength‑based, trauma‑informed, and youth development practices to support positive outcomes and a safe therapeutic environment.
