Job summary

EPIC Health in Chicago, IL is seeking a Family Nurse Practitioner (FNP) who is bilingual in English and Polish. The role is on-site, full-time, with a competitive salary and bonuses.

Responsibilities include comprehensive patient care, developing treatment plans, collaborating with physicians, ordering diagnostics, and providing preventive care in a fast-paced clinic. Requires IL APRN/FNP license, Master's in Nursing, and board certification.

Qualifications

• Active Illinois APRN/FNP license required.
• Master's degree in Nursing or related field required.
• Board certification as a Family Nurse Practitioner required.
• Bilingual in English and Polish required.

Responsibilities

• Provide comprehensive patient care: Examine patients, obtain health histories, perform physical examinations, and assess patient conditions to develop appropriate diagnoses and treatment plans.
• Develop treatment plans: Diagnose and manage common acute and chronic conditions and prescribe medications and therapies within the scope of Illinois APRN practice.
• Collaborate with physicians: Work closely with physicians and other healthcare professionals to coordinate comprehensive patient care.
• Order and interpret diagnostics: Order, administer, and interpret diagnostic testing, including laboratory work, EKGs, and imaging studies, as appropriate.
• Provide preventive and holistic care: Counsel patients on health promotion, disease prevention, growth and development, family planning, and preventive healthcare.
• Perform clinical procedures: Provide services such as injections, suturing, wound care, immunizations, and other procedures within scope of practice.
• Coordinate patient care: Follow up on patient needs, referrals, diagnostic results, and ongoing treatment plans.
• Support the clinical team: Work collaboratively with medical assistants, technicians, physicians, and other members of the healthcare team to ensure efficient patient care.
