Job summary

Buckner Children and Family Services Community: Family Hope Center seeks a bilingual Family Coach/Case Manager to interview clients and assess needs, provide coaching, and connect families to community resources. The role supports individuals facing poverty, addiction, abuse, homelessness, and related challenges.

Responsibilities include designing program activities, coordinating services, and maintaining enrollment and case notes while collaborating with community partners.

Qualifications

• Bachelor’s Degree in Social Work or related field required.
• Spanish language proficiency required for serving Spanish-speaking clients.
• Minimum of 2 years case management experience with at‑risk children and families.
• Must be able to drive and meet Buckner insurance requirements.
• Physical capability to lift up to 50 pounds and stand/walk for extended periods.

Responsibilities

• Create and implement Family Hope Center programs aligned with approved core programs.
• Coordinate supportive services and plan patient-centered activities for families.
• Assist with enrollment, intake, and assessment documentation.
• Provide case management, counseling, and referrals as needed.
• Develop relationships with community partners to coordinate services for clients.
