JusticeWorks YouthCare is hiring a Family Advocacy Case Manager to support families as they navigate social service and legal systems in York County, PA. You will serve as a primary point of contact, assess needs, connect families with housing, healthcare, and counseling resources, and help achieve stability.

This full-time, salaried role requires a high school diploma, valid driver's license, and the ability to pass background clearances and drug screening, with evenings and weekends required

Qualifikationen

• High school diploma required.
• Valid driver’s license, auto insurance, and reliable access to a vehicle.
• Ability to pass all required background clearances.
• Ability to pass pre-employment, post-offer drug screening.
• Willingness to work non-traditional hours, including evenings and weekends.
• Fluency in English and Spanish preferred.

Aufgaben

• Provide crisis intervention, case management, and advocacy for families navigating social service and legal systems.
• Connect families with housing, financial assistance, healthcare, and counseling resources.
• Monitor family progress, provide ongoing support, and adjust service plans as needed; maintain accurate documentation and meet program requirements.
• Transport clients using your personal vehicle as necessary.
