Job summary

Synergy Family Services in Hyattsville, MD, seeks an outpatient mental health therapist (contractor) to provide individual and family therapy within the OMHC program, focusing on engagement, treatment planning, documentation, and care coordination. Qualifications

• Master’s degree in social work or related field.
• Maryland license (LMSW/LCSW/LGPC/LCPC) or equivalent.
• Knowledge of ethical, trauma-informed, culturally responsive practice.
• Ability to assess risk, maintain confidentiality, seek consultation.
• Reliable access to technology and ability to use an EHR.

Responsibilities

• Provide individual and family therapy using trauma-informed, culturally responsive approaches.
• Maintain caseload and ensure frequency per treatment plan; coordinate with the team.
• Complete documentation within deadlines using agency systems.
• On-site at Langley Park Service Center at least two days per week; telehealth as available.
• Coordinate care with Psychiatry, case management, schools, and community partners.
• Address safety concerns and complete risk assessments as needed.
