Job summary

NLACRC in California seeks a Consumer Services professional to provide information, advocacy, and service arrangement for consumers and families. You will participate in developing program plans, identify and coordinate services, and document according to NLACRC policies.

The role requires coordinating with interdisciplinary teams, attending meetings, and advocating for individuals served, including home or community settings.

Qualifications

• Bachelor’s degree in psychology, social work, sociology, or related human services field and at least two years of related experience.
• A Master’s degree in a related subject may be substituted for experience.
• Understanding of family systems and person-centered planning; empowering individuals to achieve goals.
• Knowledge of intellectual and developmental disabilities, good organizational skills.
• Excellent verbal and written communication skills; proficiency with MS Word and MS Outlook.
• Service coordination is community-based and requires home/community/office meetings; valid CA Driver’s License and reliable transportation; bilingual Spanish may be required.

Responsibilities

• Participation in developing program plans for individuals; monitor and revise plans as needed.
• Identify and coordinate services for individuals and families.
• Documentation, forms and reports per regulations and NLACRC policies.
• Advocate for individuals served by community agencies.
• Schedule and attend interdisciplinary meetings with individuals served and families.
• Identify barriers to service delivery and implement culturally sensitive strategies; work in diverse settings.
• Rotate officer-of-the-day duties with other coordinators; assist coworkers with projects; provide emergency on-call services when required.
