Job summary

Family Health Centers at NYU Langone in Brooklyn is seeking a Community Health Worker to join an interdisciplinary team serving School Based Health Centers and community partners. The role focuses on engaging patients and caregivers, linking to social services, and promoting preventive care.

Responsibilities include screening for social drivers of health, assisting with benefits and housing, advocating for access, maintaining records, and coordinating with school and community staff to enroll

Qualifications

• High School Diploma or GED required.
• At least 1 year of health service experience.
• Bi-lingual English/Spanish or Chinese, Arabic, Russian or Yiddish.
• Computer literate with basic office skills.

Responsibilities

• Screen caregivers for social drivers of health.
• Assist patients and families with benefits, entitlements, and housing.
• Advocate for patients when barriers to care exist (language, literacy, transportation, insurance).
• Provide education to patients and caregivers on preventive health.
• Advocate for access to services indicated in education plans.
• Collaborate with school and community staff to coordinate services for families.
• Increase enrollment in School Based Health Center.
• Maintain electronic records and complete documentation on time.
