Planned Parenthood Northern California is seeking a Temp Bilingual Community Health Worker to support Solano County communities at Fairfield and Vallejo Health Centers, with a flexible schedule including evenings and weekends.

The CHW will act as a liaison to improve access to reproductive health services, provide health education, resource navigation, and care coordination while addressing social determinants of health.

Qualifikationen

• Bilingual English/Spanish proficiency may be required depending on center
• Experience in community outreach or patient navigation preferred
• Ability to travel within service area to health centers and events

• Build trusting relationships with patients using culturally responsive, trauma-informed, patient-centered care
• Identify barriers to healthcare access and connect patients with resources
• Coordinate with internal and external resources and care teams to support referrals
• Assist patients with MyChart enrollment and scheduling appointments
