• Bilingual proficiency required in English and another language.
• Bachelor’s degree in Public Health, Health Education, or a related field.
• Alternatively an associate degree with 2 years relevant experience.
• Or a high school diploma/GED with 4 years relevant experience.
• Valid driver’s license required.

Responsibilities

• Provide community outreach, health education, referrals, and care coordination.
• Support medical case management and follow up with assigned participants.
• Coordinate educational and disease-prevention activities.
• Build relationships with healthcare providers, schools, daycares, clinics, libraries, and community organizations.
• Connect participants with appropriate healthcare and community resources.
• Maintain accurate program documentation and assist with reports and presentations.
• Collaborate with nurses, case managers, public health educators, and community partners.
• Conduct home or community visits as required.

• Candidates must meet one of the following:
• Bachelor’s degree in Public Health, Human Services, Health Education, Community Health, or a related health field; or
• Associate degree in a related field plus 2 years of relevant experience; or
• High school diploma/GED plus 4 years of relevant public health or health-related experience.
• Valid driver’s license required
