Job summary

Community Health Partnership of Illinois is seeking a Community Health Worker in Champaign, IL to engage with migrant and underserved populations. The role focuses on linking residents to primary care, social services, and preventive health resources.

You will collaborate with providers, PHNs, and partners to educate on COVID-19 practices, disease prevention, and accessing services, while maintaining culturally responsive communications.

Qualifications

• High school diploma or equivalent.
• Bilingual Spanish/English required.
• Strong communication and interpersonal skills.
• Proficient in Microsoft Office applications.
• Ability to multitask and prioritize tasks effectively.

Responsibilities

• Work out in the community with target populations and partners to meet COVID-19 and related deliverables.
• Link individuals to primary care and identified social services in collaboration with CHP's teams and partners.
• Educate patients on COVID-19 adherence, health initiatives and chronic disease prevention and management; promote healthy habits.
• Assist patients with securing medical appointments and connecting to insurance and transportation to visits.
