• High School Diploma or GED required.
• Minimum of 2 years of experience in health care or community outreach.
• Bilingual English/Spanish/Mandarin/Cantonese preferred depending on location.
• Valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Assist patients with health care navigation, referrals, and scheduling.
• Provide culturally appropriate health information and support.
• Address social determinants of health and connect to relevant resources.
