Job summary

CMCS (Cardinal McCloskey Community Services) is seeking a Community-Based Service Provider to deliver direct services to children and their caregivers enrolled in CFTSS and HCBS programs. You will develop personalized treatment plans using a person-centered planning approach and work within a multidisciplinary team to align services with treatment goals.

Qualifications

• Bachelor's Degree in Human Services or related field is required.
• Two years of experience working with children to support their development and wellbeing.
• Knowledge of mental health diagnoses, symptomatic behaviors, and trauma responses.
• Flexible schedule including evenings and weekends.
• Bilingual (English-Spanish) preferred.
• Strong communication (oral and written) and technology skills.
• Effective engagement skills; creative and resourceful.

Responsibilities

• Provide goal-driven, direct services to assigned youth and caregivers.
• Develop and implement treatment plans, safety plans, and service-related documentation.
• Collaborate with other providers to coordinate services that support the child's and family's health and wellbeing.
• Track and monitor service hours for a diverse caseload.
• Work within a multi-disciplinary team and ensure services align with treatment goals.
