Job summary

Cardinal McCloskey Community Services (CMCS) is seeking a Community-Based Service Provider to deliver direct services to children and their families within CFTSS and HCBS programs. You will develop person-centered treatment plans, coordinate with schools and healthcare providers, and support youths in achieving goals while maintaining thorough documentation.

Qualifications

• Bachelor’s Degree in Human Services or related field.
• Two years of experience working with children to support their development and wellbeing.
• Knowledge of mental health diagnoses, symptomatic behaviors, and trauma responses.
• Flexible schedule (including evenings and weekends).
• Bilingual English-Spanish required.
• Strong communication (oral and written) and technology skills.
• Effective engagement skills; creative and resourceful.

Responsibilities

• Provide direct, goal-focused services to assigned youth and caregivers.
• Develop and implement person-centered treatment plans based on the strengths and needs of the child and family.
• Assist youth and families in identifying and achieving treatment goals.
• Complete treatment plans, safety plans, progress notes, and all required documentation accurately and within established timelines.
• Submit service-related notes within 48 to 72 hours of service delivery.
• Coordinate services and collaborate with schools, healthcare providers, community agencies, and other supports involved in the family's care.
• Participate in interdisciplinary team meetings and case consultations.
• Maintain a service calendar documenting all scheduled, completed, missed, or rescheduled services.
• Communicate effectively with Managed Care Organizations and other stakeholders as required.
• Attend clinical supervision, service-based trainings, and all mandatory agency trainings.
