Job summary

Lyon-Martin Community Health Services seeks a Clinical Social Worker (Licensed or Associate) who is bilingual in Spanish/English to join a collaborative, multidisciplinary team in San Francisco. 32–40 hours weekly, with up to one remote day per week.

The CSW provides case management, crisis intervention, and referrals to medical, mental health, and social services, ensuring culturally responsive care for a diverse patient population.

Qualifications

• Graduation from an accredited school of social work.
• Licensed in California as an LCSW or ASW.
• Bilingual Spanish/English preferred.
• Experience with Bay Area public benefit resources and networks.

Responsibilities

• Conduct comprehensive intake assessments to identify patient needs and social determinants of health.
• Develop individualized care plans addressing physical, mental, and social well-being.
• Coordinate care with patients, families, and care team; ensure shared consent and understanding.
• Provide crisis intervention, safety planning, and referrals to wraparound services.
• Maintain timely documentation in the EHR (Epic) and ensure follow-up care.
