Division Description

Felton Institute’s Early Psychosis award-winning program (re)MIND (formerly PREP) was originally developed in partnership with the University of California San Francisco (UCSF). Felton’s (re)MIND is an early psychosis coordinated specialty care model for early detection and treatment of schizophrenia spectrum disorders. (re)MIND core evidence-based practices include Cognitive Behavioral Therapy for Psychosis (CBTp), Strength-Based Care Management, Multi-family Group Therapy, Peer Support, Algorithm-Based Medication Management, and Individual Placement and Support (IPS) model of supported education and employment. The (re)MIND model has two additional components, Felton BEAM and BEAM UP, for individuals diagnosed with bipolar or mood disorders with psychosis and for individuals at clinical high risk of developing psychosis, respectively. Funded through partnerships with county behavioral health departments, the Felton Early Psychosis Division operates outpatient coordinated specialty care for early psychosis programs in San Francisco, Alameda, San Mateo, and Marin, and offers excellent training and workforce development.

Job Description

The Bilingual Clinical Care Manager I works as part of a multidisciplinary team and is responsible for coordinating participant care within the team and with community partners. This includes evaluating the strengths and needs of participants; identifying treatment goals collaboratively with participants and team members; and providing therapeutic interventions that target psychosis, mood, and additional issues that may impact recovery (such as trauma, substance use, interpersonal problems, and environmental stressors). The Bilingual Clinical Care Manager I is expected to provide services to participants in both the office and in the community (homes, schools, libraries, etc.). The Bilingual Clinical Care Manager I reports directly to the (re)MIND Program Manager.

Specific Responsibilities

  1. Meet regularly with assigned participants and their families in the community and at the office.
  2. Conduct assessment interviews and diagnostic evaluations with participants and parents to develop care plans.
  3. Provide evidence-based treatment intervention, including Family Psycho-Education, Multi-Family Groups, Case Management, CBT, DBT, MI, and Crisis Intervention.
  4. Assist participants with accessing needed services that may include medical, benefits, employment, housing, etc.
  5. Collaborate closely with community providers; make referrals to other community and mental health agencies.
  6. Maintain clinical records within 24 hours of each participant contact or service, in accordance with mandated quality assurance standards for MediCal and Medicare billing as well as in compliance with County Behavioral Health documentation standards.
  7. Meet all deadlines and comply with agency's documentation requirements and productivity standards.
  8. Work as part of a multidisciplinary team. Attend and contribute to team meetings and case conferences, providing participant updates, discussing clinical issues and exchanging ideas.
  9. Attend all scheduled trainings and follow requirements to maintain competency in evidence-based practices, as indicated.
  10. Engage in outreach events, including psycho-educational presentations, open houses, and provide information about early intervention for psychosis to other mental health providers and the general public as needed.
  11. Occasional evening and weekend availability may be required.
  12. Perform administrative tasks (i.e. chart completion and review, paperwork, etc.).
  13. Answer intake and referral calls.
  14. Perform other duties as assigned.

Minimum Qualification

  1. Master’s Degree (or higher) in Social Work or Marriage and Family Therapy or related field.
  2. Licensure with the California Board of Behavioral Sciences preferred or registered with the California BBS required.
  3. Two years of experience providing mental health services to youth or young adults with serious mental health challenges.
  4. Direct service experience in an early psychosis program preferred.
  5. Personal or family experience of overcoming mental health challenges highly desirable.
  6. Demonstrated proficiency in Microsoft Word, Excel, PowerPoint, Outlook, and ability to type 45 WPM.
  7. Valid California Drivers’ License with an excellent driving record.
  8. Bilingual/Bicultural and fluent in Spanish or threshold Asian languages required.

Competencies

  1. Knowledge of County Behavioral Health, community resources, and TAY population preferred.
  2. Knowledge of Cognitive Behavioral Therapy (CBT) and treatment of co-occurring mental health and substance use disorder preferred.
  3. Experience with CBT for Psychosis (CBTp) a plus.
  4. Strong assessment skills with reference to DSM-5 and well-developed interviewing skills.
  5. Ability to establish trusting, hopeful relationships with participants and their families.
  6. Openness to learning and implementing evidence-based practices to fidelity required.
  7. Demonstrated experience implementing evidence-based practices to fidelity preferred.
  8. Demonstrated ability to interact with and respect differences of opinion, beliefs, culture, appearances, and ways of life.
  9. Customer service perspective and strength-based approach to treatment.
  10. Knowledge of County, State, and Federal requirements for documentation and reporting, including Medi-Cal/Medicare standards, CPT codes, etc.
  11. Excellent communication (written and verbal) and interpersonal skills.
  12. Adaptable, open to change, and able to tolerate ambiguity.
  13. Strong organizational skills, attention to detail, ability to prioritize tasks, and effective time management.
  14. Demonstrated experience as a team player with a welcoming attitude.

FLSA Status: Exempt

Salary Range: $90,000 - $105,000 Annually (DOE)
